Back at Work…

Finally, I’m back at work, it seems like I’ve been of for a long time, even though it’s only been two days. I guess coming out of the ICU when having four hours of free time every three days was a lot, forty-eight hours is an eternity!

Tonight was a pain in the ass to put it mildly. Three psychiatric evaluations, none of which I could do anything for. One woman wanted someone to come to her house to “make” her husband go to the doctor. Sorry, lady, that’s not my job. One was a 15 year old who showed up because she was mad at her grandmother. Sorry, kid, you have to go back home. the last was the only lady that really needed help. Unfortunately, the health plan that carries the name of our hospital will not pay for psychiatric admissions to that self-same hospital. Patients are sent elsewhere despite extensive histories of psychiatric illness and years of treatment here. So after hours playing phone tag getting her insurance company to approve an admission at a hospital one hour away, the family left, threatening to sue. I made them sign a discharge form saying that they KNEW there was an emergency bed available in another hospital. Between those three patients alone, probably half of my shift was consumed.

the rest of tonight’s patients are run of the mill. Belly pain, chest pain, rectal bleeding…you know, the usuals. No great new insights on my end as a physician in training except that I’m really slow, patients like me, and in the meantime, stacks of charts pile up of patients waiting to be seen. Paperwork awaits…
