Thrilling conclusion

Here is a slice from the abdominal CT from the hematemesis patient. We were concerned about an aorto-enteric fistula and did IV contrast only. Note the two bright spots in the middle of the circularish lesion with an air/fluid level and air bubbles in it. What the heck is that??? CT scan with IV contrast [...]

Hematemesis, Part I

60ish year old man, sent in by his PCP to our ER for hemetemasis. PCP thinks he’s probably got a GI bleed, and notes in his clinic visit, “risks include smoking & prednisone”. The patient had been to another ED nearby the night before for chest pain, lots of labs, EKGs and xrays done with [...]

Interesting Cases

You never want to be labeled, “an interesting case”. And frankly, there are very few interesting cases left. By this time in my residency, not to mention for the staff docs, pretty much 100% of the patients you see suffer from some variation of an “ordinary” disease. Sure, some are more exciting than others and [...]

Mad as a Hatter

Blind as a bat,Red as a beet,Dry as a bone,Hot as a hare,Mat as a hatter. I loved CSI the very first time I saw it, because in the episode, Grissom recited this poem while his accomplices gazed at him in awe. As a medical student, it just seemed like a funny poem to remember, [...]

Track marks, friends and a handful of pills

A car of what appear to be college aged students pulls up in front of the ER just as I’m walking past the entry way. A tech rushes in to grab a stretcher and I follow it out to the car. I see two strapping young men pull their buddy out of the car by [...]

Flight 028

Failed suicide attempt. Man in his 50s, feels like the black sheep of his family, ties a rope around his neck, climbs a ladder, ties the rope to the cieling rafters, kicks the ladder out from underneath himself. Rope breaks, man falls onto concrete floor, hits his head, suffers skull fracture. Nephew finds him, calls [...]

The Traveling Story

If you havn’t seen it yet, check out the Traveling Story from Dr. Charle’s site, a collaborative, semi-fictional story of a medical intern turned schizophrenic following a pilgramage to rescue desert animals from the heat…really, there is much more to it than that. written by seven wonderful medical bloggers. CHeck it out.

Indoor dangers!

I got stung by a wasp today INSIDE My house! The kitties were looking at something in the corner, underneath a towel. I picked up the towel to show them there was nothing scary underthere, and that they were both bigger than IT. There was nothing there. Suddenly, I felt a sharp STING in my [...]
